Course Description: System complexity growth is unrelenting and the unfortunate consequences are that the application of historical preventive tools like FMEA, FTA, QFD, etc. are no longer able to effectively discover and remove potential critical failure modes.. This highly complex environment is termed Open Context, meaning that the boundaries of the investigation space are virtually undefined. The short course will introduce two advanced concepts, The first being Systems Theoretic Process Analysis (STPA) and the second being Directed Acyclic Graphs (DAGs). By integrating these two approaches an extensible analysis can be undertaken that includes a new way of visualization and new language by which to express it. This course will be an introduction to this unique approach and will reveal the benefits and tradeoffs that accompany it. Recommendations on implementation will also be addressed as it is a radically different approach and will require changes to already fortified cultural conventions.
